In order to reduce the amount of contact necessary to complete paperwork for your pet's visit to our clinic, you'll be asked below to fill in all of the information we'll need in order to have your proof of vaccines ready & available to you after your visit.

Once you have completed your form you'll see the BOOK IT! button which will take you to the payment form. You may pay using your Paypal account, or a credit/debit card processed thru Paypal card services. Until your payment is processed you're appointment is not reserved. Please allow for the page to load fully or you may lose your appointment slot and you will have to complete the form all over again.
You will receive an email at the address you provide. This email will confirm your appointment time, services, and instructions. Please check your inbox, spam and junk mail folders for this email and keep it for your records. You do NOT have to print the form. This email will also contain a cancellation link so that if for some reason you need to cancel your appointment you can do so at your convenience, and it also allows another individual to then use that appointment. In order to receive a refund for your appointment, you must cancel your appointment at least 24 hours before your appointment is scheduled to happen. Appointments cancelled during the 24 window will not be eligible for a refund.

Puppies and kittens can receive a distemper and/or bordetella vaccine as young as 8 weeks old. They CANNOT receive a rabies shot until they are at LEAST 4 months old. If your pet is not at least 4 months old please do not request a rabies shot for them - they will not be able to receive the shot.

** Due to supply issues and increased national demand we are unable to guarantee availability of the Canine Influenza vaccine or the price of the vaccine if it can be acquired. We will make information available on the day of the clinic if the vaccines are available and you will have the opportunity to add that vaccine to purchase that vaccine for your pets appointment if you wish.
The form below is dynamic, meaning that as you answer certain questions, other questions or options may appear, so please complete the form from top to bottom. In order to complete your paperwork accurately, EVERY field on the form is required to be completed. You will need to complete the form separately for each pet needing vaccines.
There is a $2.25 processing fee to register each pet for the vaccine clinic. This fee allows for staffers to complete paperwork in advance so that your appointment is quicker and with minimal contact. This fee is not refundable if you cancel your appointment.
